About The Position

The Broderick Street Adult Residential Facility (BSARF) is a community-based home for individuals who struggle with mental health, physical, and medical challenges. Residents are provided with board & care including meals, medication management, nursing and counseling services, social activities, and daily living support.  This is a unique opportunity for an experienced, kind, and compassionate professional to work in a milieu community setting with a complex, multi-cultural client population.  BSARF is an innovative partnership between RAMS, a non-profit community-based organization, SFDPH, Community Behavioral Health Services, and Housing & Urban Health.  Position:  Licensed Vocational Nurse (LVN) Type: Per Diem/Hourly Location: 1421 Broderick St, San Francisco, CA  94115  Job Summary:  The Licensed Vocation Nurse (LVN) provides clinically & culturally competent nursing services on-site to adult residents, offers milieu support, and participates in social activities with residents to further support engagement.

Requirements

  • Completion of an accredited LVN nursing program.
  • Current valid CA LVN license.
  • One year of psychiatric nursing experience providing care for dual diagnosed patients with both medical & mental health issues.
  • Ability to safely lift &/or move patients.
  • Health, TB, and background (Live Scan fingerprinting) clearances; First Aid, CPR, and any other mandatory State/Federal requirements upon hire.

Nice To Haves

  • One year of acute care experience, highly preferred.
  • Familiarity with SF community behavioral health systems of care and community resources/services.
  • Strong commitment to the philosophy, goals, & mission of RAMS in providing community-based, culturally competent, and consumer-guided mental health services.
  • Commitment to engaging in professional development activities relevant to treatment strategies for the population of BSARF.
  • Strong interpersonal communication skills, ability to function independently, and interface professionally with a diverse, multi-cultural, and interdisciplinary staff at all levels.
  • Bilingual skills in threshold languages of Mandarin or Cantonese are highly preferred in order to meet the needs of the residents served by this position.

Responsibilities

  • Admittance of patients, including RN assessment, transcribing orders, ordering meds from pharmacy, etc.
  • Provide medication monitoring, administering injections, dispensing of medications, and phoning in refills to the pharmacy.
  • Collaborate with Nurse Manager & Medical Director to assure and maintain adequate medical equipment, supplies, medications, and related items required for the program.
  • Provide nursing treatments to clients as required or as ordered by primary care physician (may include diabetes management).
  • Perform and complete relevant clinical and administrative documentation, and to submit them in a timely manner.
  • Maintain client records according to standards of the clinic.
  • Participate in multidisciplinary treatment meetings and staff meetings as instructed by supervisor.
  • Participate in quality improvement programs, therapeutic treatment groups, and other activities as assigned
  • Other duties as assigned.
