Licensed Vocational Nurse (LVN) - Per Diem - Assisted Living
About The Position
We’re looking for a compassionate and dependable Licensed Vocational Nurse (LVN) to join our assisted living team. In this role, you’ll provide basic therapeutic, restorative, and preventive care to residents while supporting their independence and overall well-being. You’ll collaborate with residents, families, physicians, and caregivers to implement individualized care plans and ensure the highest quality of care. The LVN also provides direction and supervision to care staff in accordance with community policies and state regulations. Requirements
- Current licensure by the California Board of Vocational Nurse and Psychiatric Examiners
- One year of experience as a Licensed Vocational Nurse in assisted living, long-term care- or a similar setting
- Strong communication, clinical, and leadership skills
- Compassionate and patient-centered approach to care
- Ability to work collaboratively in a fast-paced environment
- All Employees must be able to pass background checks (fitness for duty physical, fingerprinting, employment references) as required by a licensed residential care facility.

Responsibilities
- Advocating patience, cheerfulness and a positive work environment.
- Being patient, level headed and cool under pressure.
- Provides initial emergency medical response to residents needing assistance.
- Initiates and coordinates requests for emergency medical care, including contacting physicians, ordering emergency (911) or non-emergency ambulance transfers, and initial assessments of resident medical needs.
- Administers medications to residents as ordered.
- Administers treatments, including dressing changes, as ordered.
- Provides direct nursing care to residents within the guidelines of RCFE regulations and facility policy.
- Documents nursing care in support of planned outcomes.
- Consults with the Director of AL/SCR regarding desired patient outcomes and progress or lack of progress toward the outcomes.
- Identifies changes in resident status that interfere with the ability to meet basic needs.
- Identifies problems and reports to the Director of AL/SCR for modification of the plan of care.
- Observes and assesses effects of medications, reports findings to Director of AL/SCR and documents appropriately.
- Attends AL Department meetings as scheduled.
- Responsible for completion and e-mailing of shift report.
- Completes all online training scheduled by the Director of Staff Development and Director of AL/SCR.
- Assists in the effective operation of Casa de las Campanas and participates in relevant facility activities.
- Responsible for assisting Director in the evaluating of AL CNAs
- Orders and maintains medications.
- Keeps accurate maintenance of medical records.
- Assist with prepping paperwork for any discharge of residents from Health Center back to AL/SCR.
