About The Position

The Licensed Vocational Nurse (LVN) provides direct patient care and clinical support within Enhanced Care Management (ECM) and street-medicine programs serving individuals experiencing homelessness and other underserved populations. This role operates exclusively in community-based, mobile, and non-traditional settings (e.g., street outreach, shelters, encampments), collaborating closely with interdisciplinary teams to deliver accessible, trauma-informed care and support care coordination efforts.

Requirements

  • Graduate of an accredited Vocational Nursing program
  • Current and unrestricted California Licensed Vocational Nurse (LVN) license
  • Current BLS (Basic Life Support) certification
  • IV therapy and blood draw certification and documented experience
  • Demonstrated competency in venipuncture and blood draws
  • Valid driver’s license with the ability to drive mobile medical vehicles as required

  • Experience or strong desire to work with individuals experiencing homelessness, substance use disorders, serious mental illness, or other vulnerable populations

  • Strong documentation skills and comfort working in electronic health records in field-based settings

Responsibilities

  • Deliver direct patient care in community-based and street-medicine settings in accordance with LVN scope of practice, ECM requirements, and organizational protocols
  • Assist licensed providers with patient examinations, treatments, and procedures conducted in non-traditional settings
  • Administer approved medications and vaccinations as ordered by licensed providers
  • Perform venipuncture and blood draws for laboratory testing, including proper specimen handling, labeling, and transport
  • Perform routine clinical procedures, including: Obtaining and documenting vital signs
  • Collecting laboratory specimens and performing CLIA-waived tests as ordered
  • Wound care, dressing changes, and basic first aid
  • Monitor patients for adverse or allergic reactions following medication or treatment administration and initiate emergency measures per protocol
  • Conduct health assessments and screenings during street outreach, shelter-based events, and mobile medical operations
  • Provide patient education related to medications, disease prevention, harm reduction, and basic health maintenance
  • Handle, store, and manage medications and vaccines in compliance with CDC, OSHA, and organizational policies
  • Accurately document medical and psychosocial histories, assessments, treatments, medications, and outcomes in the electronic health record in support of ECM and street medicine activities
  • Collaborate with ECM team members, including community health workers, case managers, and social workers, to support care coordination and continuity of care
  • Advocate for the healthcare needs of individuals experiencing homelessness and work to reduce systemic barriers to accessing care
  • Maintain accurate, timely, and complete documentation to support ECM compliance, reporting, and care coordination requirements
  • Ensure strict adherence to HIPAA and patient confidentiality standards in field-based environments
  • Support patient flow during outreach and street medicine operations and communicate delays or changes to team members and patients as appropriate
  • Coordinate internal and external referrals to medical, behavioral health, and community-based resources
  • Ensure medical equipment, supplies, and medications are stocked, organized, and ready for field deployment
  • Manage clinical supplies and inventory, including restocking and usage tracking
  • Assist with prescription coordination, including refill requests and pharmacy communication, within LVN scope of practice
  • Transport clinical and operational supplies to outreach sites and points of care as required
  • Attend required staff meetings, conferences, trainings, and organizational meetings
  • Perform other duties as assigned to support ECM and street-medicine operations
