Requirements

  • Licensed Vocational Nurse in State where they are working.
  • Should have good communication skills.
  • CPR certification current.
  • Valid driver license.
  • Ability to work well with others, function well under pressure, and function independently.
  • Must comply with the standards outlined in the Nurse Practice Act.
  • Acts only with the scope of their state license or state certification or registration.
  • LVN must keep their qualifications current at all times.
  • Current State License to practice in New Mexico as a Licensed Vocational/Practical Nurse

Nice To Haves

  • Language: Bilingual English/Spanish preferred

Responsibilities

  • Provides direct patient care as defined in State’s Name Nurse Practice Act
  • Implements plan of care initiated by the registered nurse.
  • Provides accurate and timely documentation consistent with the plan of care.
  • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
  • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
  • Uses equipment and supplies effectively and efficiently.
  • Participates in personal and professional growth and development.
  • Performs other duties as assigned by the registered nurse.
  • Maintains verbal and/or written communications among other personnel involved in the care of the patient; maintains clinical records on all patients referred to physical therapy and in consultation with the licensed physical therapist.
  • Performs and submits required documentation of patient’s clinical notes, progress reports, summaries and case conferences along with changes in orders.
  • All patient visit notes must be turned in the next scheduled work day, not to exceed forty-eight (48) hours.
  • Advances professional growth; participates in training sessions at least 2 times annually; attends short courses, and studies professional journals and texts.
