Hayward Gardens Post Acute is seeking a skilled LVN to join our clinical team for the NOC Shift. In this role, you will provide direct nursing care and serve as a supervisor for our nursing assistants, ensuring our residents receive the highest standard of care throughout the night. Requirements

Must possess a valid California Licensed Vocational Nurse (LVN) License.
Strong understanding of post-acute care, medication administration, and resident assessment.
Capable of supervising and guiding a team of nursing assistants effectively.
Excellent verbal and written communication skills in English.
Reliability for the full-time NOC shift schedule.

Responsibilities

Provide high-quality nursing care to residents in accordance with physician orders and individual care plans.
Oversee and supervise the day-to-day nursing activities performed by nursing assistants during your shift.
Ensure all care is delivered in accordance with current federal, state, and local standards and facility regulations.
Assess resident conditions, manage medications, and perform treatments as required, maintaining the highest degree of quality care at all times.
Work under the direction of the Director of Nursing Services or Nurse Supervisor to ensure seamless care transitions and shift hand-offs.
Maintain accurate and timely clinical records and report any significant changes in resident status.
