Hayward Gardens Post Acute is seeking a skilled LVN to join our clinical team for the NOC Shift. In this role, you will provide direct nursing care and serve as a supervisor for our nursing assistants, ensuring our residents receive the highest standard of care throughout the night. Requirements

  • Must possess a valid California Licensed Vocational Nurse (LVN) License.
  • Strong understanding of post-acute care, medication administration, and resident assessment.
  • Capable of supervising and guiding a team of nursing assistants effectively.
  • Excellent verbal and written communication skills in English.
  • Reliability for the full-time NOC shift schedule.

Responsibilities

  • Provide high-quality nursing care to residents in accordance with physician orders and individual care plans.
  • Oversee and supervise the day-to-day nursing activities performed by nursing assistants during your shift.
  • Ensure all care is delivered in accordance with current federal, state, and local standards and facility regulations.
  • Assess resident conditions, manage medications, and perform treatments as required, maintaining the highest degree of quality care at all times.
  • Work under the direction of the Director of Nursing Services or Nurse Supervisor to ensure seamless care transitions and shift hand-offs.
  • Maintain accurate and timely clinical records and report any significant changes in resident status.
