About The Position

We're seeking compassionate, dependable, and licensed LVNs to join our dedicated team for AM and NOC 12-hour shifts. If you're looking to make a meaningful impact in a supportive skilled nursing environment, this opportunity is for you.

Requirements

• Current and valid California LVN license
• CPR certification required
• Graduate of an accredited LVN program
• Ability to work independently and manage multiple priorities
• Strong written and verbal communication skills
• Must be able to lift up to 25 lbs and perform physical job duties as required

Nice To Haves

• Knowledge of Medicare/Medi-Cal regulations preferred
• Experience with PointClickCare (PCC) preferred

Responsibilities

• Direct and monitor the day-to-day functions of nursing assistants
• Ensure quality care in accordance with resident care plans
• Admit, transfer, and discharge residents as needed
• Administer medications and treatments as prescribed
• Monitor and report changes in residents' conditions
• Accurately complete and maintain residents' medical records
• Record physician orders, chart treatments, and update care plans
• Prepare required forms, evaluations, and reports
• Provide guidance and oversight to CNA staff
• Conduct rounds to ensure quality of care and staff compliance
• Communicate with residents, families, physicians, and interdisciplinary teams
• Participate in staff meetings, in-services, and training sessions
• Ensure adherence to infection control, safety, and facility protocols
• Participate in audits, surveys, and quality assurance programs
• Maintain confidentiality of resident and facility information
