About The Position

Under the supervision of the Director of Nursing, the Licensed Vocational Nurse (LVN) provides nursing care to residents as prescribed by the physician and in accordance with applicable nursing standards and regulatory requirements and supervises Certified Nursing Assistants (CNAs) in the delivery of direct resident care.

Requirements

  • Must be a Licensed Vocational Nurse (LVN) in good standing and currently licensed by the state.
  • Must be able to supervise other employees and work cooperatively to obtain resident care objectives.
  • Must be able to interact positively with residents, families, and regulatory agencies, and work collaboratively with employees at all levels.
  • Must be able to maintain consistent attendance and be willing to work long or irregular hours, often under pressure.
  • Must comply with all local health regulations and successfully complete a post-offer health assessment.
  • Must be able to perform the essential functions of the position with or without reasonable accommodation.
  • Ability to read, write, and speak fluent English to communicate effectively with residents, families, staff, and community agencies.
  • Ability to perform physical activities such as walking, reaching, bending, stretching, pulling, and lifting.
  • Adequate fine and gross motor coordination to carry out required tasks.

Responsibilities

  • Review and maintain medical records and nursing notes; assess residents’ physical and emotional status.
  • Monitor resident activity and provide nursing care in accordance with physician orders, care plans, nursing standards, and facility policies.
  • Coordinate and assist with resident admissions, transfers, and discharges.
  • Transcribe and carry out physician telephone orders accurately.
  • Communicate effectively with residents, family members, interdisciplinary team members, and management regarding resident status.
  • Prepare and administer medications as ordered and in compliance with nursing standards and facility policies.
  • Complete all charting and documentation accurately, timely, and in accordance with established policies and procedures.
  • Understand, comply with, and promote resident rights.
  • Participate in the development, implementation, and revision of individualized care plans.
  • Participate in in-service education, orientation programs, and job skills training to maintain professional competence.
  • Monitor the care environment and nursing practices to ensure compliance with safety, fire, disaster preparedness, infection control, and departmental policies.
  • Supervise Certified Nursing Assistants (CNAs), including assigning duties, monitoring performance, and providing coaching or corrective feedback as needed.
