Communicates with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family. Document all services provided. Demonstrates knowledge of Medicare/Medicaid regulations for home health benefits. Report changes in patient condition and needs to RN Great time management skills

Requirements

Current Texas LVN license
Minimum of 2 years as a Licensed Vocational Nurse (LVN)
Have dependable transportation, drivers license, and car insurance

Nice To Haves

Home Health experience preferred

Responsibilities
