Sequoia Transitional Care is a beautiful, welcoming 99-bed skilled nursing facility seeking a dedicated LVN Charge Nurse for our full-time NOC shift. Requirements

Active California LVN license in good standing
Current CPR certification
Must be able to provide recent medical clearance for SNF work
Must pass background check and be eligible to work in the U.S.

Nice To Haves

SNF or post-acute experience preferred, but not required
New graduates encouraged to apply – training provided
Strong communication and documentation skills
Comfortable using EMR systems
Compassionate, dependable, and team-oriented

Responsibilities

Provide direct nursing care in accordance with individualized resident care plans
Serve as Charge Nurse during the NOC shift, overseeing CNAs and ensuring quality care
Administer medications and treatments as prescribed
Monitor residents for changes in condition and promptly report to RN Supervisor or physician
Accurately document care and observations in the EMR system
Communicate effectively with residents, families, and the interdisciplinary team
Maintain compliance with infection control, safety standards, and facility policies
Respond to emergencies and follow established protocols
Participate in shift handoffs, care planning, and training activities
