Requirements
- Active Texas LVN license in good standing. (Required)
- Strong clinical judgment, organization, and communication; reliable and team-oriented.
Nice To Haves
- 1+ year of nursing experience in SNF/LTC preferred (new grads welcomed with strong references).
Responsibilities
- Assess & plan: Perform assessments and contribute to individualized care plans.
- Medications & treatments: Administer per orders; monitor responses and side effects.
- Clinical monitoring: Track vitals, changes in condition, and escalate promptly.
- Communication & education: Support residents and families with clear, timely updates.
- Teamwork: Collaborate with RNs, CNAs, therapy, and providers for smooth transitions of care.
- Documentation: Complete accurate, and timely notes in the EMR.
- Professional growth: Participate in ongoing training and competencies.
