Requirements
- Valid CA Licensed Vocational Nurse License
- Maintain a current CPR certification
- 2+ years of experience working in direct patient care
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
Responsibilities
- Implement the plan of care as developed and directed by the Registered Nurse
- Provide accurate, thorough, and timely clinical documentation consistent with the patient’s plan of care and agency standards
- Collect assessment data and perform observations, promptly reporting changes in patient condition to the supervising RN and/or physician
- Participate in the coordination of home health services by collaborating with the interdisciplinary care team to ensure seamless, patient‑centered care
- Use medical equipment, supplies, and resources safely, effectively, and efficiently
- Engage in ongoing professional development, maintaining clinical competency and participating in training or in‑service education as required
