About The Position

Sunnyvale Gardens Post Acute is looking for compassionate, dependable, and skilled Licensed Vocational Nurses (LVNs) to join our dedicated care team. As an LVN at Sunnyvale Gardens Post Acute, you will play a vital role in supporting residents’ recovery and overall well‑being. Requirements

  • Active California LVN license
  • Current CPR/BLS certification
  • Strong clinical and communication skills
  • Ability to multitask and maintain accuracy in a fast‑paced environment

Nice To Haves

  • Previous experience in post‑acute, skilled nursing, or long‑term care preferred but not required

Responsibilities

  • Provide high‑quality nursing care to residents under RN and provider supervision
  • Administer medications and treatments in accordance with care plans
  • Monitor and document resident health status, reporting changes promptly
  • Assist with assessments, care planning, and resident education
  • Ensure compliance with all facility, state, and federal guidelines
  • Promote a positive, respectful, and compassionate care environment
