Licensed Vocational Nurse (LVN) – Hospice

We’re growing and looking for an experienced Licensed Vocational Nurse (LVN) to join our Hospice Care team! As a Hospice LVN , you will provide direct patient care under the supervision of a Registered Nurse (RN). You’ll follow the plan of care, deliver treatments, and collaborate with the interdisciplinary team to achieve positive patient outcomes.
Requirements
- Active CA LVN License
- Minimum 1 year of LVN experience (hospice experience preferred)
- Current CPR certification
- Valid driver’s license and insured, reliable vehicle
- Ability to work independently and as part of a team

Responsibilities
- Implement nursing care based on comprehensive assessment and plan of care
- Document patient services accurately and timely
- Educate patients and families on diagnosis and self-care
- Coordinate hospice services and report needs to the care team
- Manage pain and symptoms; evaluate patient response to medications
- Use equipment and supplies effectively
- Participate in quality improvement and professional development
