You’ll follow individualized care plans, administer treatments, and collaborate with the interdisciplinary team to ensure comfort and dignity for every patient. Schedule: Nights and weekends

Requirements

  • Valid California LVN license
  • Minimum 2 years of hospice experience
  • Current CPR certification
  • Valid driver’s license and insured, reliable vehicle

Nice To Haves

  • Experience with Home Care Home Base (HCHB) preferred

Responsibilities

  • Implement nursing care based on comprehensive assessments and care plans
  • Accurately document patient services to reflect the plan of care
  • Educate patients and families on diagnoses and self-care plans
  • Coordinate hospice services with the interdisciplinary team
  • Use equipment and supplies effectively and efficiently
  • Provide pain and symptom management, evaluating patient response
  • Participate in quality improvement and professional development activities
