About The Position

GrandCare Health Services is a leader in post-surgical and orthopedic rehabilitation, delivering expert, compassionate care in the comfort of our patients’ homes. We proudly serve communities throughout Southern California with a strong commitment to dignity, respect, and exceptional patient outcomes. If you're an LVN seeking a supportive, clinician-focused environment—not the typical home health caseload—GrandCare offers a unique opportunity to specialize in orthopedics, neuro, and pain management while making a real impact. The Licensed Vocational Nurse (LVN) provides high-quality patient care in the home setting by following the established plan of care under the supervision of the Registered Nurse. This role is ideal for LVNs who enjoy one-on-one care, patient education, and helping individuals recover after orthopedic and surgical procedures. GrandCare is a physician-trusted leader in post-surgical rehabilitation delivered in the comfort of your own home. We are recognized by the Centers for Medicare and Medicaid Services as a 5-STAR agency, placing us in the top 10% nationally. Our experienced clinicians work closely with your physician to develop a personalized plan of care specific to your needs. Our tailored, concierge approach is proven to promote faster healing, reduce stress and anxiety, and get you moving again. GrandCare Health Services is an Equal Opportunity Employer. We consider all qualified applicants without regard to race, color, religion, sex, national origin, disability, veteran status, or other protected characteristics. The employer for this position is stated in the job posting.
