The Licensed Vocational Nurse (LVN) II provides a variety of administrative, clinical nursing, health education, and outreach services within the community health center. Basic skills for physical assessment include observation, auscultation, palpation and percussion. Basic mental status assessment includes observation of general behavior, appearance and attitude, collection of data relative to sensorium or orientation to time, person and place, and assessment of recall ability. When the LVN notes a problem, complication or abnormality, he/she must notify the physician or Registered Nursing Supervisor under whose direction the licensee functions. The LVN cannot independently evaluate or synthesize data. The LVN II have additional lead duties. Lead Duties: Provides advanced nursing care while leading and mentoring clinical staff. Oversees patient care coordination, delegates tasks based on scope of practice, supports staff development through training and mentorship, and collaborates on quality improvement and audit readiness. Serves as a clinical resource and ensures adherence to policies and procedures.
