The Community Licensed Vocational Nurse (LVN) plays a vital role in delivering high-quality, patient-centered care within Pacific Health Group. Under the direct supervision of a Registered Nurse (RN), the LVN provides nursing care to individuals and families in their homes, community clinics, and other designated outreach locations. This role focuses on promoting health, preventing illness, and managing chronic conditions through education, assessment, and direct care, while adhering to all established policies, procedures, and regulatory requirements. Collaboration with Lead Care Managers (LCMs): LVN will conduct multiple on-site visits each month in patients’ homes, shelters, or community centers accompanied by lead care managers (LCMs) assigned to cases. LVN will use these visits to establish trust, gather first-hand insights, and address concerns right away. Close collaboration with LMCs is crucial for the success of this role. Example: While visiting a member recovering at home, you might discover that they lack mobility aids—prompting you to collaborate with the assigned lead care manager to arrange for durable medical equipment and coordinate in-home physical therapy.
