About The Position

The Licensed Therapist (LPC or LICSW) is responsible for providing compassionate, evidence-based mental health services to clients within long-term care and outpatient settings. This position plays a vital role in supporting the emotional, psychological, and behavioral well-being of individuals by delivering therapeutic interventions, completing assessments, and collaborating with interdisciplinary teams to ensure coordinated, person-centered care.

Requirements

  • Active, unrestricted Alabama license as a Licensed Professional Counselor (LPC) or Licensed Independent Clinical Social Worker (LICSW).
  • Master’s degree in Counseling, Social Work, or related field required.
  • Demonstrated ability to provide person-centered therapy and maintain professional boundaries.
  • Strong written and verbal communication skills.
  • Ability to work independently while collaborating as part of a multidisciplinary team.
  • Ability to travel to assigned facilities and/or clinic locations as needed.
  • Ability to sit, stand, and move throughout the workday, including light lifting (<25 lbs).
  • Maintain professional appearance and demeanor in all settings.

  • Experience in facility-based, geriatric, or behavioral healthcare preferred.

Responsibilities

  • Conduct comprehensive biopsychosocial assessments and develop individualized treatment plans.
  • Provide individual, group, and family therapy sessions using clinically sound, evidence-based approaches.
  • Deliver therapy services within nursing homes, assisted living facilities, and/or outpatient environments.
  • Support clients experiencing a wide range of mental health concerns including depression, anxiety, trauma, grief, and adjustment issues.
  • Maintain timely, accurate, and confidential documentation in the electronic medical record (EMR) in accordance with HIPAA and company policy.
  • Collaborate closely with psychiatric providers, nursing staff, and facility leadership to ensure continuity of care.
  • Participate in multidisciplinary treatment team meetings and case consultations.
  • Provide crisis intervention and follow-up as needed to ensure client safety and stability.
  • Monitor client progress and adjust treatment plans as clinically indicated.
  • Engage in ongoing professional development and continuing education to maintain licensure and enhance clinical practice.
