Country Manor's Rapid Recovery & Aquatic Center is now hiring for a full-time Licensed Social Worker! The LSW will assist us in maintaining our distinctive level of care by collaborating with residents/patients and their families to develop plans of care, monitor progress, and ensure success. This is a full-time position, working primarily daytime between 8:00 a.m. and 5:00 p.m. It includes a rotation of every fourth weekend and on-call holidays.

Requirements

Must hold a current MN license from the Board of Social Work

Responsibilities

Collaborate with the multidisciplinary healthcare team
Interview residents/patients/representatives and assess their current condition, needs, strengths and weaknesses
Adhere to and follow the Social Work Code of Ethics and Standards of Practice
Develop and implement resident/patient-centered plans of care
Monitor and evaluate clients’ progress and modify treatment plans accordingly
Maintain accurate records and report on clients’ status
Refer clients to community services and assist with coordination of needs to ensure successful discharge plans
