The LSW will assist us in maintaining our distinctive level of care by collaborating with residents/patients and their families to develop plans of care, monitor progress, and ensure success. This is a full-time position, working primarily daytime between 8:00 a.m. and 5:00 p.m. It includes a rotation of every fourth weekend and on-call holidays.

Requirements

  • Must hold a current MN license from the Board of Social Work
  • Social perceptiveness and empathy

Responsibilities

  • Collaborate with the multidisciplinary healthcare team
  • Interview residents/patients/representatives and assess their current condition, needs, strengths and weaknesses
  • Adhere to and follow the Social Work Code of Ethics and Standards of Practice
  • Develop and implement resident/patient-centered plans of care
  • Monitor and evaluate clients’ progress and modify treatment plans accordingly
  • Maintain accurate records and report on clients’ status
  • Refer clients to community services and assist with coordination of needs to ensure successful discharge plans
