About The Position

This position is responsible for providing direct service to the residents and their families under the supervision and direction of the Director of Social Services. This position will provide assistance in addressing psychosocial issues in realistic, dignified and satisfying manner and to provide on-going support and encouragement to the residents and family members.

Requirements

  • Must possess a bachelor’s degree in Social Work (BSW) or a human services field
  • 1-3 years of Long Term Care Facility experience preferred but not required
  • Computer literacy.

Responsibilities

  • Participate in the development of the inter-disciplinary care plan and MDS+ assessment tool.
  • Prepares quarterly progress notes, initial assessments and MDS+’s as assigned by the director.
  • Participate in the quarterly resident review meetings.
  • Participate in the Room Change committee and help to facilitate room changes
  • Participate in department meetings and in-services.
  • Work as a liaison between Social Services and other departments within the facility.
  • Assist in the psychosocial evaluation of residents upon admission to the facility as well as in the development of an initial assessment of each resident.
  • Work with families and residents as needed.
  • Schedule medical appointments and arrange transportation for residents.
  • Assist Residents in developing pre-need funeral arrangements.
  • Serve as resident rights advocate.
  • Must possess the ability to weigh the facts, make decisions and know when to seek assistance or consultation with others.
  • Must have a strong understanding and/or commitment to confidentiality in themselves and others.
  • Must be able to be creative; plan and implement ideas and evaluate the results.
  • Must be able to foster and maintain a positive working relationship with other departments as well as within the Social Service Department.
  • Must be able to work independently with minimum supervision or guidance.
  • Other duties as assigned
