About The Position

MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve. Licensed Social Worker (LCSW or LBSW) - Adult Day Health Locations: Gaithersburg, Maryland Schedule: Monday–Friday | Day Shift | Full-Time & Part-Time Available Compensation: LCSW: $70,000/year + Mileage Reimbursement LBSW: $55,000–$60,000/year + Mileage Reimbursement Make a Difference Every Day We’re seeking Licensed Social Workers (LCSW or LBSW) to join our Adult Day Health Program in a case management oversight and coordination role. This position focuses on monitoring services, supporting case coordinators, and ensuring high-quality care delivery, helping participants remain safely in their homes and communities.

Requirements

  • LCSW: Master’s Degree in Social Work (MSW) from an accredited program
  • Active Maryland LCSW license
  • LBSW: Bachelor’s Degree in Social Work (BSW) from an accredited program
  • Active Maryland LBSW license
  • Reliable transportation to travel between program sites
  • Strong organizational, communication, and leadership skills
  • Ability to manage multiple cases and support team members effectively

Responsibilities

  • Provide oversight and guidance to Case Coordinators, ensuring care plans align with participant needs and program requirements
  • Review, monitor, and support the development of individualized service and care plans
  • Conduct psychosocial assessments and reassessments as required by program standards
  • Monitor participant progress and outcomes; recommend updates to goals and services as needed
  • Ensure documentation, authorizations, and care plans are accurate, complete, and submitted on time
  • Serve as a liaison among participants, families, case coordinators, physicians, and interdisciplinary team members
  • Participate in interdisciplinary team meetings and provide clinical input as needed
  • Support compliance with state, federal, and organizational regulations
  • Assist with problem resolution, care coordination, and service transitions
  • Perform related duties to support program operations and quality outcomes
