About The Position

We are currently seeking a licensed social worker plays a key role in enhancing the quality of life for residents by supporting their psychosocial well‑being, coordinating services, and ensuring smooth transitions throughout their stay. This position works closely with nursing, therapy, admissions, and families to advocate for resident needs and uphold regulatory standards.

Requirements

  • Bachelor’s Degree in Social Work (BSW) or related human services field.
  • Licensed Social Worker (LSW) in the State of Ohio OR eligibility to obtain licensure.
  • Knowledge of long‑term care regulations and discharge planning procedures.
  • Strong communication, documentation, and organizational skills.
  • Ability to work compassionately with older adults and families in stressful situations.

Nice To Haves

  • Experience in long‑term care or skilled nursing facility (SNF) setting.
  • Familiarity with MDS Section D, care planning, and EMR systems.

Responsibilities

  • Complete psychosocial assessments, care plans, and documentation in accordance with state and federal regulations.
  • Provide emotional support, counseling, and crisis intervention to residents and families.
  • Coordinate discharge planning, including community resources, home care, and support services.
  • Participate in interdisciplinary team (IDT) meetings and contribute to individualized care planning.
  • Maintain accurate and compliant documentation within electronic medical records (EMR).
  • Assist residents with adjustment to facility life, including addressing concerns and advocating for resident rights.
  • Facilitate family meetings and communicate care updates professionally and compassionately.
  • Support admissions by welcoming new residents and assisting with required documentation.
  • Work collaboratively with departments to ensure a positive resident experience.
  • Uphold confidentiality, ethical practices, and all regulatory standards.
