Requirements
- A valid social work license is required for this role.
Responsibilities
- Assessing the psychosocial needs of residents and developing individualized care plans to address their emotional, social, and financial concerns.
- Providing counseling and support to residents and their families, offering guidance and assistance in navigating healthcare systems and community resources.
- Collaborating with interdisciplinary teams to coordinate resident care and facilitate communication between residents, families, and healthcare providers.
- Advocating for residents' rights and preferences, ensuring their voices are heard and respected in decision-making processes.
- Maintaining accurate and confidential resident records, documenting assessments, interventions, and progress notes in compliance with regulatory requirements.
- Conducting regular meetings and educational sessions for residents and staff on topics related to mental health, aging, and social services.
- Monitoring and evaluating the effectiveness of social services programs and initiatives, implementing improvements as needed to enhance resident outcomes.
- Ensuring compliance with all state and federal regulations governing social services in long-term care settings.
