About The Position
The Licensed Social Worker (MSW) provides direct patient care and care management services across the continuum of care. This role collaborates with multidisciplinary healthcare teams to assess patient needs, coordinate care, support discharge planning, and promote safe transitions of care. The Social Worker serves as a patient advocate while ensuring compliance with state and federal regulations, including CMS Conditions of Participation. This position supports patients of all ages through holistic assessment, counseling, crisis intervention, referral coordination, and education to achieve optimal health outcomes and appropriate utilization of healthcare resources.
Requirements
- Master’s Degree in Social Work (MSW) from an accredited program
- Current Social Work license from the Ohio Counselor and Social Work Board
- Minimum of one (1) year of social work experience
Nice To Haves
- BLS (Basic Life Support)
- Acute care or hospital-based social work experience
- Knowledge of discharge planning and care transitions
- Strong communication and collaboration skills
- Ability to work independently and within interdisciplinary teams
- Knowledge of community and post-acute care resources
