About The Position

The Social Worker provides social services to patients facing substantial and often challenging medical issues and a large variety of social factors. Referrals may be given for needs, and it will be the responsibility of the Social Worker to follow through with appropriate referrals for counseling, treatment, transportation, financial assistance and other services needed. The Social Worker provides assistance with services for children, adults, and their families. The Social Worker is responsible for all mental and behavioral health referrals. This position is a key member of the Medication Assisted Treatment team. The Social Worker coordinates care with support from referring providers, primary care provider, and community resources. The position also may conduct follow up case consultation with Primary Care and/or Mental Health Providers regarding client compliance and/or services.

Requirements

  • Bachelor’s Degree in Social Services or related field
  • Licensed in the State of Minnesota

Responsibilities

  • Coordinate and connect patients for diagnostic assessments and neuropsychological assessments
  • Assist with referrals/connections to agencies like WIC, County public assistance programs, MNsure, housing resources, food programs, in-home services, support groups, transportation resources, utility programs, renter’s assistance and refunds, etc.
  • Assist with collection of urine drug abuse screens (UDAS) per policy
  • Attend trainings with the team to promote cohesion and continued program development
  • Provide social worker interventions, support and consultation to physicians, providers, nurses, and patients on an as-needed basis regarding psychosocial issues, bioethical issues, etc.
  • Conduct and document assessment of patient and family needs, communicate care issues, and develop appropriate plan
  • Perform psychosocial assessments and planning for high-risk patients
  • Participate in patient care planning responsive to age specific needs
  • Document all patient interactions within referrals and/or the patient’s chart
  • Exhibit good communication with multidisciplinary team
  • Attend and participate in pertinent internal and community meetings
  • Receive referrals for children, adults, and their families in need of care coordination for mental and/or behavioral health concerns
  • Contact patient/family to discuss referral, schedule appointments in accordance with referral, obtain necessary release of information, collect insurance information and confirm Medical Assistance eligibility and provide additional related information to patients upon request
  • Establish and maintain consistent contact with patients enrolled in Medication Assisted Treatment (MAT)
  • Openly communicate and collaborate with MAT Physicians and RN Navigator to ensure precise care coordination for MAT patients
  • Serve as backup to RN Navigator and complete patient intake assessment and/or initial visits for MAT inquiries and new MAT patients
  • Perform all other related duties as assigned in a professional and responsive manner
