About The Position

The Licensed Social Worker (LSW) provides behavioral health services for clients across Ohio, including children, adolescents, adults, couples, and families. Services include but are not limited to community linkage, skills building, problem solving, and social support. Services may be delivered in the office, in clients’ homes, or in community‑based settings.

Requirements

  • Current Licensed Social Worker (LSW/LPC) licensure in the state of Ohio
  • Minimum of a bachelor's degree in social work or a related field with 1-2 years of experience
  • Experience working with children and adolescents in a behavioral health or social services setting
  • Experience providing evidence‑based mental health interventions
  • Experience using electronic health records (EHR) systems preferred
  • Local travel of approximately 10–15% is required; reliable transportation and a valid driver’s license are necessary
  • Ability to remain in a stationary position for extended periods while completing documentation or attending meetings
  • Ability to move through various environments, including navigating stairs, uneven surfaces, and confined spaces during field visits
  • Ability to operate standard office equipment (e.g., computer, phone, copier)
  • Ability to communicate effectively with clients, families, and interdisciplinary team members in person, by phone, and in writing
  • Ability to occasionally lift or carry items up to 25 pounds (e.g., case files or outreach materials)
  • Sufficient visual acuity to read and interpret documents, charts, and electronic records
  • Ability to maintain composure, professionalism, and sound judgment in unpredictable or emotionally intense situations

Responsibilities

  • Provide evidence‑based mental health counseling services to individuals, couples, and/or families
  • Provide therapeutic services to children, adolescents, and teens, as well as adults and older adults, in accordance with best practices and developmental needs
  • Conduct comprehensive clinical assessments
  • Prepare, implement, and evaluate the effectiveness of individualized treatment plans
  • Conduct risk assessments and provide crisis intervention services as needed
  • Utilize an electronic health records (EHR) platform for clinical documentation and care coordination
  • Complete clinical documentation accurately and in a timely manner, in accordance with organizational and regulatory requirements
  • Connect clients to appropriate community resources and provide ongoing skills‑building, problem‑solving, and social support
  • Collaborate effectively as a member of a multidisciplinary and diverse care team
  • Demonstrate strong communication, problem‑solving, and interpersonal skills
  • Maintain the flexibility and professionalism required to work with a diverse client population
  • Learn, follow, and uphold Carex standards, policies, and best practices
