About The Position

Orchard View Post Acute is seeking a compassionate, licensed Social Worker to support residents and families through every stage of post-acute care. This role is essential in helping ensure safe discharges, coordinating community resources, and providing emotional support in a fast-paced skilled nursing environment. Requirements

  • Active Social Work license in the state of Idaho
  • Strong knowledge of discharge planning and community resources
  • Excellent communication, organization, and documentation skills
  • Ability to work collaboratively with clinical and administrative teams

Nice To Haves

  • Experience in skilled nursing, post acute, or long-term care preferred

Responsibilities

  • Complete psychosocial assessments and develop care plans in collaboration with the interdisciplinary team
  • Provide counseling, crisis intervention, and emotional support to residents and families
  • Coordinate discharge planning, including placement, resources, and follow-up needs
  • Assist with resident advocacy, care conferences, and family communication
  • Support compliance with federal/state regulations and facility policies
  • Maintain accurate, timely documentation in accordance with standards and guidelines
