About The Position
Orchard View Post Acute is seeking a compassionate, licensed Social Worker to support residents and families through every stage of post-acute care. This role is essential in helping ensure safe discharges, coordinating community resources, and providing emotional support in a fast-paced skilled nursing environment. PACS is revolutionizing the delivery and leadership of post-acute healthcare and support services across the country. Requirements
- Active Social Work license in the state of Idaho
- Strong knowledge of discharge planning and community resources
- Excellent communication, organization, and documentation skills
- Ability to work collaboratively with clinical and administrative teams
Nice To Haves
- Experience in skilled nursing, post acute, or long-term care preferred
Responsibilities
- Complete psychosocial assessments and develop care plans in collaboration with the interdisciplinary team
- Provide counseling, crisis intervention, and emotional support to residents and families
- Coordinate discharge planning, including placement, resources, and follow-up needs
- Assist with resident advocacy, care conferences, and family communication
- Support compliance with federal/state regulations and facility policies
- Maintain accurate, timely documentation in accordance with standards and guidelines
