Reporting to the Clinic Manager, the Licensed Clinical Social Worker (LCSW) provides comprehensive psychosocial support, crisis intervention, counseling, education, and care coordination for patients and families coping with acute and chronic hematologic conditions, with a primary focus on Sickle Cell Disease.

Requirements

  • Master’s Degree in Social Work (MSW) from an accredited institution
  • Active California Licensed Clinical Social Worker (LCSW) license
  • Minimum one (1) year of experience in a healthcare setting
  • Experience working with children, families, and adult populations
  • Basic Life Support (BLS) certification

Nice To Haves

  • Experience working with chronic illness populations, particularly Sickle Cell Disease or hematology
  • Bilingual in English and Spanish

Responsibilities

  • Conduct comprehensive psychosocial assessments and deliver culturally sensitive, developmentally appropriate interventions for pediatric and adult patients and their families
  • Provide supportive counseling, psychotherapy, and crisis intervention, particularly for patients managing chronic pain and unpredictable disease crises
  • Coach patients and families on emotional coping skills, resilience, and pain management strategies
  • Advocate for patients’ autonomy, wellness, and access to appropriate care and community resources
  • Collaborate with a multidisciplinary care team to develop and implement individualized, holistic care plans
  • Coordinate care and connect patients with community resources including schools, regional services, Social Security, rehabilitation programs, transportation, and support services
  • Provide education and consultation to patients, families, and care teams regarding psychosocial needs and disease impact
  • Assist with case management, program development, and educational resource maintenance
  • Maintain timely and accurate documentation in the electronic medical record in accordance with policy and regulatory standards
  • Support mission-aligned projects, trainings, seminars, and community initiatives as needed
