Requirements

• Prefer 2 years of social work experience in a long-term care or hospital setting.

• Licensed Social Worker in the State of Texas.

Responsibilities

• Assist in planning, organizing, implementing, evaluating and directing the overall operation of the Social Service Department.
• Provide medically related social work services to residents.
• Conduct resident, family, and physician interventions.
• Deliver psychosocial services to residents.
• Coordinate vision, hearing, and dental services for residents.
• Perform assessments and care planning for residents.
• Facilitate discharge planning for residents.
