Communicates/collaborates with the work team using a multidisciplinary approach. Provides linkage to community resources to facilitate optimal care. Provides age specific care to patients/families in all age groups. Develops and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs. Maintains a current working knowledge of UH network services and promotes utilization of such services. Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers. Additional Responsibilities Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.

Requirements

  • Master's Degree from accredited school of Counseling Required
  • Attainment of LPCC status is encouraged with one year of eligibility. (Required proficiency)
  • Demonstrates ability to interact professionally with physicians, nurses and other members of the healthcare team. (Required proficiency)
  • Demonstrates ability to apply theoretical and clinical knowledge in delivery of care. (Required proficiency)
  • Multi tasks and prioritizes work. (Required proficiency)
  • Performs autonomously as a professional. (Required proficiency)
  • Excellent verbal and written communication. (Required proficiency)
  • Licensed Professional Counselor (LPC) (Required)

Nice To Haves

  • Hospital/Health Agency experience (Preferred)
  • LPCC status within one year of eligibility (Preferred)
  • This outpatient therapist will be seeing adult patients with various mood disorders, including depression, anxiety, trauma, etc. They will also see adolescents with dual diagnoses. Must be proficient in performing diagnostic assessments, treatment planning, counseling, providing resources, documentation, and have IOP experience. Experience with DBT, group and individual are preferred. The position is a hybrid: 3 days in office and 2 days remote. Will be required to work 4:00pm-7:00pm, 3 days/week to work with the adolescent IOP group.
