About The Position

UVA Health seeks a Licensed Clinical Social Worker or Licensed Professional Counselor to join the Virtual Primary Care On Demand Clinic as a Behavioral Health Consultant (BHC), an integrated member of the virtual primary care team. This is a fast-paced role that uses targeted interventions to address the behavioral health needs of a rapid-access primary care population and maintain availability for same-day virtual consultation. The BHC focuses on population health impact and building provider capability to address behavioral health in a fully virtual environment. The Behavioral Health Consultant will work as a member of the primary care team, providing consultation-based behavioral health services to patients in collaboration with their primary care providers in a Clinical Practice Group office. These duties include 1) operating in a consultative role within a primary care team utilizing an integrated behavioral health model of care; 2) providing recommendations regarding behavioral interventions to the referring primary care provider; and 3) conducting focused interventions with referred patients on behalf of the referring primary care provider. About This Position This position is based in the Virtual Care Clinic, a new UVA Health initiative providing rapid-access primary care to patients across Virginia who do not have an established primary care provider. The clinic is staffed by a team of three Advanced Practice Providers (APPs) rotating to provide seven-day coverage. The BHC will operate as an integrated team member providing same-day virtual warm handoffs and scheduled video consultations, with typical patient contact of 1-5 visits focused on empowering patients for self-management. Virtual Delivery Requirements Must reside in Virginia and deliver all care from within the Commonwealth Reliable high-speed internet required for video-based patient care Computer equipment will be provided Private, confidential workspace suitable for clinical telehealth Schedule 32 patient-facing hours per week plus 8 hours of administrative time (total 40 hours) Schedule will include weekend and/or evening coverage as part of the rotation to support seven-day clinic operations Flexibility is essential to success in this role

Requirements

  • Master's degree in social work from a CSWE accredited social work program or a master's degree in counseling from a CACREP or CORE accredited counseling program is required.
  • Minimum of 3 years of experience providing independent patient care required.
  • Licensed to practice as a Licensed Clinical Social Worker or Licensed Professional Counselor in the Commonwealth of Virginia (LCSW, LPC) or applicants with an active license from another state must obtain Virginia licensure.
  • BLS certification required (within 90 days of start date).
  • Must reside in Virginia and deliver all care from within the Commonwealth
  • Reliable high-speed internet required for video-based patient care
  • Private, confidential workspace suitable for clinical telehealth
  • Flexibility is essential to success in this role

  • Demonstrated comfort with interventions appropriate for focused contacts (e.g., Motivational Interviewing, Solution-Focused Brief Therapy, Cognitive-Behavioral Therapy, Acceptance and Commitment Therapy)
  • Experience managing clinical flow in time-limited settings
  • Ability to rapidly assess and intervene (triage or crisis experience is valuable)
  • Proficiency with EHRs and integrated documentation
  • Comfort with telehealth technology and virtual patient engagement

Responsibilities

  • Provides access to focused assessment and appropriate collaborative care and treatment in the primary care setting, including same-day consultation visits requested by the primary care team
  • Conducts risk assessments as indicated to support primary care providers managing crisis situations
  • Delivers evidence-based interventions to patients experiencing mental health symptoms, behavioral health issues, chronic disease management, as well as for the promotion of general health
  • Practices collaborative, stepped and measurement-based care, including appropriate follow-up, that reflects a population health-influenced approach for the primary care population
  • Collaborates with primary care team members to develop clinical pathways and psychoeducational classes for select priority populations
  • Contributes to patient education materials used by practice for commonly seen behavioral health conditions and problems
  • Contributes to the optimal functioning of primary care teams through collaborative decision support and interdisciplinary consultation that is mindful of other team members' roles and workflows
  • Educates primary care team in the basic principles of brief behavioral and cognitive behavioral interventions and reinforces their use in the medical visit
  • Supports medication adherence and efficacy
  • Assists with triage, care coordination and access to outside resources for patients as appropriate
  • Participates in any relevant clinical training, quality improvement activities, and staff meetings
  • Maintains clinical records and other necessary paperwork in a timely manner to comply with all administrative regulations
  • Maintains virtual visibility and availability to the primary care team during clinic hours through team communication platforms
  • Is available for curbside consultation (brief provider-to-provider interaction via chat or phone)
  • Delivers all patient care via HIPAA-compliant video telehealth platform
