About The Position

Merakey is launching a brand-new program in Baton Rouge, Louisiana, and seeking a Licensed Practical Nurse (LPN) to help shape its success. This role is perfect for a professional passionate about holistic healthcare, bridging mental and physical health services, and working within a multidisciplinary team.   We’re seeking a compassionate and organized Licensed Practical Nurse (LPN) to join our team as a Targeted Care Manager, providing person-centered care coordination for individuals with complex medical and behavioral health needs. In this role, you'll be instrumental in promoting whole-person wellness by improving access to services, supporting care transitions, and addressing social determinants of health. Starting rate of $24.04 hour + mileage reimbursement Location: 730 Colonial Drive Baton Rouge, Louisiana 70806 This is a meaningful opportunity for an LPN passionate about whole-person care and community health. Requirements

  • Must hold a current and unrestricted Licensed Practical Nurse (LPN) license in the state of Louisiana.
  • Minimum of 2 years of related work experience.
  • Experience working with individuals living with serious mental illness (SMI), substance use disorders (SUD), or co-occurring disorders is highly preferred.

Nice To Haves

  • Lived experience with mental health and/or substance use challenges and sustained personal recovery.
  • Certified Peer Support Specialist or willingness to complete certification upon hire.
  • Strong communication, active listening, and interpersonal skills.
  • Ability to maintain appropriate boundaries and handle sensitive information confidentially.
  • Proficiency in using computers and electronic health records (EHR) preferred

Responsibilities

  • Conduct assessments and develop individualized care plans addressing medical, behavioral, and social needs.
  • Coordinate services and facilitate communication among healthcare providers, clients, and families.
  • Provide health education, medication support, and chronic condition coaching.
  • Monitor client progress, adjust care strategies, and support transitions across care settings.
  • Maintain accurate documentation and contribute to program evaluation and quality improvement.
