About The Position

Visiting Nurse Health System (Visiting Nurse) is seeking a Licensed Practical Nurse (LPN) to join our Home Health team who's not only a skilled nurse - but also a calm, compassionate, and empathetic presence caring for our patients. We are seeking two (2) individuals willing to work Monday through Friday across Decatur, GA. This is a non-exempt position. Our corporate office is located in Dunwoody, GA 30346; however, this position is headquartered out of our Hartsfield office, near the airport.

Requirements

  • LPN Graduate from an accredited school of nursing or program
  • Completion of an IV training program or certification in IV therapy
  • Experienced in advanced wound therapies
  • Current license by the Georgia State Board of Nursing.
  • Minimum of one - two (1-2) years' experience in a Medicare-certified home health agency is preferred.
  • Minimum of two (2) years as an LPN
  • Current valid Driver's License, proof of auto insurance, and a reliable vehicle

Responsibilities

  • Provide nursing care, emotional support, patient and family education regarding disease process, self-care techniques, prevention strategies, clinical nursing observations, and documentation for Home Health patients.
  • Provides nursing services as ordered by the physician and under the direction of the RN.
  • Reports subjective and objective patient findings to the RN.
  • Under the direction of the RN, obtains physician's orders and ensures orders exist for each visit made.
  • Provides necessary skills to report changes appropriately; documents pertinent information and care rendered to the patient to ensure continuity of care.
  • Demonstrates knowledge of infection control standards and techniques as per VNHS policy and procedures and consistently practices these techniques.
