About The Position

The Licensed Practical Nurse (LPN) in a home health care setting is responsible for providing direct, one-on-one care to patients in their homes. This role involves monitoring patient health, administering treatments, managing medications, and offering essential support to patients with various medical needs. Collaboration with a team of healthcare professionals, including Registered Nurses (RNs) and physicians, is required to deliver high-quality, compassionate care that improves patient well-being.

Requirements

• Trade School Graduate (Required)
• Active LPN License in Virginia (Required)
• BLS certification (Required within 90 days)
• Experience in customer service, testing, medication administration.

Responsibilities

• Provide direct, one-on-one care to patients in their homes.
• Monitor patient health and administer treatments.
• Manage medications and offer essential support to patients.
• Collaborate with healthcare professionals to deliver high-quality care.
• Serve as patient advocate in the prevention of illness or disease.
• Provide care and treatment for patients and clinical support and teaching for patient and family members.
• Implement nursing interventions and care plans.
