The Licensed Practical Nurse (LPN) plays a critical role in delivering high-quality nursing care within an assisted living facility, ensuring the health and well-being of residents during night shifts. This position requires the LPN to perform comprehensive patient care activities, including wound care management, pain assessment, and administration of treatments, while maintaining accurate documentation. The LPN will educate patients and their families on health maintenance and disease prevention, fostering a supportive and informative environment. Collaboration with interdisciplinary healthcare teams is essential to develop and implement individualized care plans that meet each resident's unique needs. Ultimately, the LPN ensures a safe, compassionate, and effective care environment that promotes patient comfort and recovery throughout the night hours.

  • Provide direct patient care including monitoring vital signs, administering medications, and performing wound care treatments.
  • Perform venipuncture and other necessary clinical procedures accurately and safely.
  • Educate patients and their families on health conditions, treatment plans, and pain management techniques.
  • Document all nursing care activities, patient responses, and changes in condition in compliance with facility policies and regulatory standards.
  • Collaborate with healthcare team members to assess patient needs and update care plans accordingly.
  • Respond promptly to patient needs and emergencies during night shifts, ensuring patient safety and comfort.
  • Maintain a clean, organized, and safe environment within the assisted living facility.
  • Valid and current Nursing License as a Licensed Practical Nurse (LPN) in the state of practice.
  • Proven experience working night shifts in a healthcare or assisted living setting.
  • Demonstrated ability to perform venipuncture and wound care procedures.
  • Proficiency in reading, writing, and speaking English to effectively communicate with patients and healthcare team members.
  • Basic knowledge of pain management principles and patient care techniques.
  • Experience working specifically in an assisted living facility or long-term care environment.
  • Additional certifications related to wound care or pain management.
  • Training or coursework in patient education and health promotion.
  • Familiarity with electronic health record (EHR) systems.
  • Strong interpersonal skills and ability to work collaboratively within multidisciplinary teams.
