The LPN is a vital member of the Assertive Community Treatment (ACT) Team. The LPN provides medication management, medication education, crisis intervention, medication administration to consumers of the ACT Team. The LPN is also responsible for following the orders of the team's prescribers to ensure the members are receiving adequate treatment and interventions. LPNs assist with PCP and other medical specialty coordination to ensure the members needs are being met. The LPN assist with assessment and the development of the person-centered treatment plan. This is community-based work.

  • Provide medication management and education to consumers of the ACT Team.
  • Administer medications as prescribed.
  • Conduct crisis intervention as needed.
  • Follow orders from the team's prescribers to ensure adequate treatment.
  • Assist with coordination of care with PCP and other medical specialists.
  • Participate in the assessment and development of person-centered treatment plans.
  • A current Louisiana Nursing License.
  • A genuine interest in working with individuals with physical and developmental disabilities.
  • Current and valid Louisiana driver's license and reliable transportation.
