Licensed Practical Nurse (LPN) - Skilled Nursing We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our skilled nursing team. Requirements

  • Valid and current Licensed Practical Nurse (LPN) license in the state of practice
  • Proven experience in a skilled nursing or long-term care setting preferred
  • Strong clinical skills and knowledge of nursing practices
  • Excellent communication and interpersonal skills
  • Ability to work effectively as part of a multidisciplinary team
  • Compassionate, patient-focused approach to care
  • Ability to handle multiple priorities and remain calm under pressure
  • Basic computer skills and familiarity with electronic health records (EHR) systems
  • Must be a Licensed Practical Nurse in good standing and be currently licensed by the Commonwealth of Pennsylvania Bureau of Professional and Occupational Affairs, Department of State; or
  • Must be knowledgeable of nursing and medical practices and procedures and laws, regulations, and guidelines governing skilled nursing facilities; and
  • Must be at least eighteen (18) years of age and completed high school or obtained the equivalent of a high school education; and
  • Must possess the ability to speak to and be understood by residents and staff, and demonstrate the ability to read, write, and follow oral and written directions; and
  • Must possess an agreeable and helpful attitude toward senior adults.

Responsibilities

  • Provide direct patient care under the supervision of registered nurses and physicians
  • Monitor and document patients' health status, including vital signs and symptoms
  • Administer medications and treatments as prescribed
  • Assist patients with activities of daily living, such as bathing, dressing, and mobility
  • Collaborate with healthcare team members to develop and implement care plans
  • Educate patients and their families on health management and post-discharge care
  • Maintain accurate and detailed patient records in compliance with healthcare regulations
  • Ensure a safe and clean environment for patients and staff
