About The Position

ChesPenn Health Services is seeking a patient-focused Licensed Practical Nurse (LPN) to support clinical care in a Federally Qualified Health Center (FQHC) setting. This position is based at the Center for Family Health in Chester, PA, a busy primary care practice providing adult medicine, pediatrics, and women’s health services. The LPN works as part of a multidisciplinary care team supporting a diverse and underserved patient population.

Requirements

  • Completion of an accredited Practical Nursing Program
  • Current, unrestricted Pennsylvania Licensed Practical Nurse license
  • Current Basic Life Support (BLS) or CPR certification
  • Knowledge of infection control, safety, and sanitation standards
  • Understanding of HIPAA confidentiality requirements and patient privacy best practices
  • Familiarity with electronic health records and clinical documentation
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively within a multidisciplinary care team
  • Compassionate, patient-centered approach with the ability to manage stress effectively
  • Current COVID-19 and influenza vaccination required

Nice To Haves

  • Prior experience as an LPN in an outpatient or community health setting preferred
  • Experience working with diverse, underserved, or high-risk populations preferred
  • Demonstrated ability to obtain and document patient histories and vital signs
  • Demonstrated ability to perform venipuncture and CLIA-waived laboratory testing
  • Demonstrated ability to administer injections and vaccinations per protocol

Responsibilities

  • Perform clinical duties under the direction of a licensed provider or Registered Nurse
  • Prepare patients for provider visits by obtaining medical history, vital signs, growth parameters, and reason for visit
  • Assist providers during examinations and procedures
  • Administer injections and vaccinations according to clinical protocols
  • Perform venipuncture and collect specimens for laboratory testing
  • Conduct visual and auditory screenings as appropriate
  • Accurately document patient care and clinical activities in the electronic health record
  • Support quality improvement initiatives and compliance with HRSA, state, and federal requirements

  • Demonstrate teamwork and professionalism in support of co-workers and organizational goals
  • Occasional evening or weekend hours may be required based on operational needs
