Licensed Practical Nurse (LPN) — Per Diem (Home Health)

We are a growing, Honolulu-based Medicare-certified Home Health agency serving patients across all of Oahu. We focus on providing high-quality care that helps local seniors remain safe, supported, and independent at home.

Requirements

• Current Hawaii LPN license
• 1+ year clinical experience (home health preferred)
• Strong communication and documentation skills
• Valid driver’s license + reliable transportation
• Ability to work independently in the field

Responsibilities

• Provide skilled nursing visits in patients’ homes under RN direction
• Assist with medication management and patient education
• Perform wound care and routine skilled nursing tasks within scope
• Communicate changes in patient condition to RN / clinical team
• Document accurately and on time
