About The Position

The Licensed Practical Nurse works as an integral member of the team providing services at agency locations across a range of service lines. The Licensed Practical Nurse is responsible for providing wellness services and health monitoring to meet the behavioral health and care issues affecting those served by the agency’s Integrated Health and Wellness programs. The primary goal of health monitoring is to identify clients at risk due to poor physical health and medical conditions, improve their health outcomes through early identification and care, and prevent lowering their quality of life or early death. Health monitoring goals include: providing health assessments and health monitoring to a wide range of agency clients, proving monitoring services to high risk clients weekly, with the aim of reducing medical hospitalizations. The Licensed Practical Nurse will be responsible for developing and implementing practices and protocols specific to the population served. Clients in crisis or with behavioral health needs (such as hoarding, depression, anxiety, substance use disorders) will be reached out to, engaged and treated by the Licensed Practical Nurse. The LPN will develop an individualized plan to address needs and include family, significant others and healthcare providers whenever possible. The primary goal of this employee is to support clients to monitor and maintain primary health goals. The Licensed Practical Nurse is part of the team and assists team members in understanding the needs of clients and help improve their quality of life.

Requirements

  • NYS Licensed Professional Nurse
  • Must have certification in behavioral health clients or 3 years of experience working with the behavioral health population.
  • Excellent time management, organizational and computer skills.
  • Ability to appropriately evaluate and intervene in crisis situations.
  • Excellent oral and written communication skills required.
  • Ability to work flexible hours (evenings/weekends) as assigned.
  • Able to travel throughout the five boroughs as needed.
  • Ability to lift up to 10lbs.
  • May be required to meet with clients and / or staff throughout programs.

Responsibilities

  • Assesses clients to establish a care plan according to their needs.
  • Perform health monitoring services and assessments
  • Responsible for tracking information/ deliverables for the Cabrini grant funds.
  • Lead workshops for clients on issues related to health and wellness.
  • Encourage clients to participate in on-going wellness and social activities.
  • Provide advocacy and education to clients
  • Facilitate coordinated care for targeted critical/high need cases.
  • Works through team approach in partnership with program staff to identify and respond to clients in need.
  • Works with client and their families to implement planning.
  • Completes documentation, both paper and electronic in a timely manner.
  • Provide counseling and therapy.
  • Provide in-service training and be available to consult with staff.
  • Conduct and prepare assessments, case logs, documents case notes, care plans for clients.
  • Refer and link clients to service providers in the local community.
  • Establish and maintain contacts with families and programs when clientss are in hospitals and rehab facilities, and helps formulate aftercare plans.
  • Responds to emergency situations and provide crisis intervention.
  • Contact and follow-up with service providers put in place for clients such as Adult Protective Services, outpatient behavioral health, substance use treatment and healthcare providers.
  • Strictly adheres to and enforces all the Agency policies regarding Confidentiality and Corporate Compliance regulations and maintains compliance with HIPAA regulations.
  • Offer time-limited groups as appropriate on such issues as loss and depression, living with illness, substance use issues and family/relationship maintenance.
  • Attend staff meetings and trainings as required
  • Participate in the development and facilitation of staff training and in-services for social service staff on relevant social service topics.
