About The Position

Working as a part of multi-disciplinary team, nurses at the Behavioral Health Crisis Center, treat and educate clients in crisis about various behavioral health and medical conditions, and provide advice and emotional support to clients and their family members/supports. Nurses provide assessment, treatment planning, education and support, medication administration and monitoring, and treatment for behavioral health and co-morbid physical health conditions while clients are on our unit. They work with the discharge team to ensure appropriate after care services and medication are accessible upon discharge. This position is located at the Behavioral Health Crisis Center. The BHCC has 30 crisis beds, eight 23-hour observation beds and a 24/7 walk-in crisis center.

Requirements

  • Candidate must possess an LPN/RN license in the state of Georgia

Responsibilities

  • Treat and educate clients about behavioral health conditions and medical conditions
  • Interview clients and/or family members using acceptable techniques that are appropriate for the clients level of functioning to obtain necessary information regarding medical, social, and/or emotional background Complete initial and ongoing Nursing assessments, treatment/discharge plans, and Orders for Services that reflect treatment goals, nursing interventions, client response, and standards of the Agency, and the State
  • Assist in performing diagnostic tests, operate medication machinery or equipment, analyze results, and consult with prescriber regarding treatment interventions.
  • Perform routine tests and take vital signs regularly as prescribed in each individual plan of care
  • Administer medications, including injectable medication, to clients as ordered and indicate in treatment plans
  • Monitor medication response, side effects and adverse effects of medications
  • Order lab-work per protocol, performs lab draws, uses CLIA-waived (Clinical Improvement Amendments) lab equipment for in-house lab-work, and obtains lab results from commercial labs; assists with referral process as needed or indicated for variations in metabolic indices
  • Record clients medical histories, symptoms, and any treatment plans in to the client record
  • Staffs complex cases with prescribers for direction; consults with other staff members related to client care
  • Provide education on diagnosis, medication use, side effects, treatment adherence, and other topics as individualized for the client.
  • Provide basic case management and linkages by making referrals for changing levels of care or to community partners
  • Complete and/or sign medication logs, progress notes, entries into medication profile, and medication and laboratory orders (within Agency established timeframes)
  • attends monthly nursing staff meetings or program meetings
  • Participates in shift change, providing information regarding client care and response
  • May be required to attend daily treatment team meetings
  • Provides appropriate information and/or input at shift change and in meetings pertaining to client care
  • Follows Prescribers Orders, Nursing Care Plans, and Treatment plans regarding services to clients and frequency of services.
  • Assists clients with activities of daily living, as needed.
  • Provides social and psychological support to clients through client engagement, listening and discussing problems and progress as necessary.
  • Utilizes skills of Safety Care to deescalate situations and avoid need for restraint.
  • Participates with team to restrain clients, when necessary and according to procedure.
  • Contacts other health care professionals within or outside the organization to obtain consumer background information in accordance with established guidelines.
  • Collaborates with service providers in addressing concerns and treatment planning changes or additions.
  • Complies with regulations on confidentiality of client information as specified by established policy.
  • Complies with all DBHDD regulations and Advantage Policies and Procedures regarding client care and any tasks/responsibilities in the operation of a BHCC.
  • Other duties as assigned by Nurse Manager, Charge Nurse, or other members of leadership.
