About The Position

Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care. The Licensed Practical Nurse (LPN) is a member of the interdisciplinary team, provides nursing care to the patient and assists the Registered Nurse with the implementation of the care plan. They provide direct patient care, collaborate with the other Interdisciplinary Team (IDT) members within the care team, and with other health care providers in various settings. The LPN provides patient care in a manner that promotes an interdisciplinary team approach, respects patients/families goals for care, meets Medicare/Medicaid conditions of participation and accreditation requirements and reflects the mission and philosophy of HospiceCare of Boulder and Broomfield Counties. Upon request, the LPN participates in all areas of clinical services.

Requirements

  • Must possess a current LPN license in the state of Colorado.
  • Must have minimum one year LPN experience
  • Must successfully pass agency required background checks.
  • Must possess a valid driver’s license, provide proof of insurance, and have reliable transportation.
  • Must comply with agency health screening requirements.
  • Must possess CPR certification or be able to obtain within 60 days of hire.
  • Must be able to demonstrate computer competency.

Responsibilities

  • Collects data regarding all body systems, signs and symptoms, deviations, acuity, vital signs, and responses in collaboration with the Registered Nurse, primary care physician, and/or hospice physician contributing to the individualized plan of care.
  • Incorporates patient/family preferences and goals within the plan of care.
  • Supports and respects the patient’s and family’s unique diversity, including spiritual and cultural beliefs.
  • Completes documentation in an accurate and complete manner as per agency standard and in compliance with regulatory standards.
  • Provides patient and family education as appropriate. Areas of education include, but are not limited to personal care, medications, procedures, safety, diet and nutrition, disease process, death and dying.
  • Contributes to the team, utilizes team resources, supports team decisions, and maintains positive working relationships with all team members.
  • Demonstrates effective verbal, written, and electronic communication skills. Gives and receives feedback in a timely open manner. Engages collaboratively in conflict resolution processes as needed.
  • Is flexible and responsive in meeting and exceeding the expectations of all customers, both internal and external.
  • Participates in mandatory educational activities and required meetings.
  • Is competent using agency information and technology systems
