Requirements

• Active LPN license in the state of Pennsylvania.
• Minimum of 1 year of experience in long-term care or rehabilitation preferred.
• Current CPR/BLS certification.
• Covid Vaccination Card
• Valid photo ID
• NIH(ASA)
• 2 Supervisory References

Responsibilities

• Provide direct nursing care to residents according to facility protocols and physician orders.
• Administer medications and treatments accurately and safely.
• Monitor, assess, and document residents’ health conditions and report any changes to the supervising RN or physician.
• Assist residents with activities of daily living, such as hygiene, mobility, and nutrition.
• Maintain accurate records of care provided and medications administered.
• Support and follow infection control and safety guidelines.
• Collaborate with RNs, CNAs, and other healthcare staff to ensure coordinated care.
