About The Position

Requirements

• Active Florida LPN license in good standing
• Graduate of an approved/accredited LPN program
• Reside in Florida with the ability to work from home in a private, secure environment
• Proficient in electronic documentation, professional communication, and patient-centered virtual care

Nice To Haves

• 1+ year experience in a clinical or care coordination role (preferred but not required)
• Experience with remote patient monitoring, care navigation, or home health
• Familiarity with EHR systems and HIPAA-compliant virtual care platforms

Responsibilities

• Provide telephonic or video-based clinical support to patients and families across a range of health concerns
• Document patient encounters, progress notes, and medication adherence in the electronic health record (EHR)
• Educate patients and caregivers on medications, care plans, and chronic condition management
• Monitor and triage patient issues, escalating concerns to RN or provider as needed
• Follow Florida Board of Nursing scope of practice for LPNs in telehealth roles
• Support care coordination activities such as appointment scheduling, referrals, and follow-up outreach
• Maintain HIPAA-compliant practices in all remote workflows
