We are currently seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our team, delivering exceptional care in patients’ homes, skilled nursing facilities (SNFs), and assisted living facilities (ALFs). As part of our team, you’ll collaborate with RNs, social workers, chaplains, and providers who share a deep commitment to high-quality care and support. Requirements
- Valid Licensed Practical Nurse (LPN) License
- Compassionate and patient-centered approach, with strong interpersonal and communication skills
- Ability to work independently and as part of a team, with excellent time management skills
Responsibilities
- Provide hands-on nursing care, including medication administration, wound care, and symptom management under the supervision of an RN.
- Support and educate patients and families, helping them understand care plans and manage symptoms effectively.
- Monitor and document patient conditions, communicating updates to the care team and adjusting care plans as directed.
- Collaborate with an interdisciplinary team, including RNs, social workers, chaplains, and CNAs, to ensure holistic care.
- Follow hospice protocols and best practices to maintain high-quality standards.
