About The Position
ExperCARE is seeking a Licensed Practical Nurse (LPN) to join our dynamic team. The LPN plays a key role in delivering high-quality nursing care, with a primary focus on geriatric patients in both long-term care and home health settings. This position involves monitoring vital signs, administering medications, and providing compassionate, individualized support. Requirements
- Current and valid Licensed Practical Nurse (LPN) license in the state of employment.
- Completion of an accredited practical nursing program.
- Experience in geriatric nursing or long-term care settings.
- Proficiency in monitoring vital signs and providing nursing care.
- Strong attention to detail in patient documentation.

Responsibilities
- Monitor and document patients’ vital signs; promptly report significant changes to supervising nurse or physician.
- Administer prescribed medications and treatments per established protocols and care plans.
- Provide direct patient care, including assistance with activities of daily living and mobility support.
- Maintain accurate and timely patient care charts to ensure continuity of care.
- Collaborate with healthcare team members to develop, implement, and evaluate individualized care plans.
- Educate patients and families on medication adherence, preventive care, and health management.
- Adapt care delivery to meet changing patient needs in long-term care and home health settings.
- Comply with healthcare regulations, safety standards, and organizational policies.
