Overview Under the direction of the Director of Nursing, the Licensed Practical Nurse (LPN) is responsible for providing quality care to residents of the assigned neighborhood. The LPN functions as a professional caregiver who is accountable for planning, implementing and evaluating all aspects of resident care, including physical, psychosocial and environmental needs.

Requirements

  • Current CT LPN license is required.
  • 2 years of full-time clinical experience in nursing is required.
  • Must be able to speak, read, and write the English language in an understandable manner.

Nice To Haves

  • Comprehensive knowledge of geriatric nursing care, principles of nursing, and physical evaluation skills is preferred.
  • Basic knowledge of human relations skills necessary to deal effectively with the geriatric residents, their families, and CHHC staff.

Responsibilities

  • Documents according to nursing practice standards.
  • Administers medications accurately and in a timely fashion and provides treatments to residents according to physician orders.
  • Provides physical comfort and a safe environment.
  • Accurately carries out physician/provider orders in a timely fashion.
  • Initiates, implements, and evaluates care plans/MDS on an ongoing basis.
  • Accurately completes admission and discharge procedures.
  • Coordinates staff and resident care appropriately to meet resident needs.
  • Follows all policies and procedures, annual mandatory in-services, required meetings, and consistently meets other conditions of employment.
  • Performs other duties as assigned.
