Requirements

  • Ongoing need for employee to see and read information, documents, monitors, identify equipment and supplies, and be able to assess customer needs.
  • Frequent interactions with providers, colleagues, customers, patients/clients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately.
  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use for typing, accessing needed information, etc.
  • Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.

Responsibilities

  • Patient Care via Telephonic Symptom Screening: Assists in the collection, reporting, and documentation of appropriate symptom data. This may include documentation of current medication information and known drug allergies.
  • Identifies patient pain or discomfort and reports and responds appropriately.
  • Notifies patients of interventions as ordered by the provider.
  • Assists physician or licensed independent practitioner with coordinating outpatient treatments or procedures as needed.
  • Documentation: Appropriately documents subjective symptom data, physician and independent practitioner orders (documented, verbal or phone that fall within the LPN's Scope of Practice) and patient and family notification and education.
  • Communication: Appropriately communicates with physicians, or licensed independent practitioner regarding symptom information, changes in patient condition, patient history data, and physician or independent practitioner orders (verbal or phone).
  • Participates in patient education.
  • Professionalism: Integrates legal and ethical standards into practice including compliance with regulatory standards, practices within scope of licensure, provides accurate and timely documentation, and understands legal implications of care delivery.
  • Demonstrates responsibility and accountability for own practice.
  • Participates in quality improvement projects and initiatives as needed.
