About The Position

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Intensive Mobile Treatment (IMT) team. As an LPN on our team, you will provide holistic care to criminal legal-involved individuals with complex mental health needs, promoting physical and mental health integration and wellness. Your expertise will be essential in conducting health screenings, administering medications, and connecting clients with primary and specialty care services. Collaborating with a multidisciplinary team, you will work to improve client outcomes and promote recovery, rehabilitation, and community integration. Requirements

  • Active New York state license as a Licensed Practical Nurse
  • At least two years of experience working with adults who have needs in the areas of mental health, substance use, trauma, violence risk, homelessness, frequent psychiatric hospitalizations, and criminal-legal involvement
  • Strong commitment to social justice and the CASES mission

Nice To Haves

  • Fluency in Spanish
  • Previous use of an electronic health record, Footholds AWARDS preferred
  • Experience in a multidisciplinary setting and field-based clinical work

Responsibilities

  • Improve the overall health of program participants, including promotion of physical and mental health integration.
  • Conduct health screenings, provide linkage to primary and specialty care, and educate on health and wellness.
  • Perform blood draws, monitor and administer psychotropic injections consistent with consumer needs preferences and practice standards, and prepare of medication to be dispensed.
  • Complete progress notes and assessments in the electronic health record within a timely manner.
  • Work with external stakeholders – such as hospitals, shelters, jails, primary care –to achieve positive client outcomes.
  • Provide services in a strength-based, recovery-oriented, trauma-informed, and culturally sensitive manner.
  • Helps maintain a safe and clean environment ensuring proper use and disposal of medical supplies/equipment as required, adheres to infection control protocols, and notifies their supervisor immediately of any potential safety hazard identified in the office/client-care environment.
