About The Position
A long-term care facility is seeking a Licensed Practical Nurse (LPN) to provide high-quality nursing care to residents in a skilled nursing and long-term care setting. The LPN will work collaboratively with the healthcare team to ensure resident safety, comfort, and overall well-being while maintaining compliance with clinical standards and regulations.
Requirements
- Active Licensed Practical Nurse (LPN/LVN) license in good standing
- Minimum of 1 year experience in a Skilled Nursing Facility (SNF) or Long-Term Care setting preferred
- Current BLS/CPR certification
- Strong communication, teamwork, and documentation skills
- Ability to work effectively in a fast-paced, team-oriented environment
- Physical ability to stand, walk, lift, and assist residents as required
Responsibilities
- Provide direct nursing care to residents in a skilled nursing or long-term care environment
- Administer medications and treatments in accordance with physician orders and facility protocols
- Monitor, assess, document, and report residents’ vital signs and changes in condition
- Perform wound care and basic medical procedures within scope of practice
- Assist residents with activities of daily living (ADLs) as needed
- Maintain accurate, timely, and compliant patient documentation
- Collaborate effectively with Registered Nurses, physicians, and interdisciplinary care teams
- Respond promptly and appropriately to emergencies and resident care needs
- Ensure adherence to facility policies, procedures, and state regulations
- Promote patient safety, dignity, confidentiality, and comfort
- Support residents’ physical, emotional, and psychosocial health
